Consent for the issue of an identity document for an Italian minor at another office

If you need to issue a travel document or an ID for a minor child at an office other than this Embassy (Questura, Commissariato, Comune, other Italian Embassies and Consulates), the non-EU parent or legal guardian of the minor child can authenticate his/her signature at our Offices.

Please note:

  • The consent is NO longer needed for the issuance of a passport to the other parent of a minor child;
  • The authentication of the signature it is not necessary for EU citizens. EU citizens can fill in the consent form, sign it and provide the original signed copy together with the copy of a valid ID to the applicant or the other parent requesting the issuance of a document for a minor child. Please note that the signature on the form has to match the signature on the valid ID provided.

If a travel document or an ID has to be issued at an office other than this Emabssy, the non EU citizens of Italian minor children can request an appointment by following the instructions provided below. The appointment will be granted by the next business day.

If you can not report to the Embassy, consent can also be given by authenticating the signature at a local notary. In this case the document will have to be apostilled. A list of notaries is provided on the page “Professionals of reference” of this website.

 

HOW TO GET AN APPOINTMENT

Send an email to consolato.telaviv@esteri.it providing the following information:

  1. Full name, place and date of birth, nationality of the parent whose signature has to be authenticated;
  2. Full name, place and date of birth of the Italian minor children to whom a document shall be issued;
  3. PEC e-mail address of the Office where the consent has to be sent to (if known);
  4. Date of the appointment (if known).

The day of the appointment, the applicant shall report to the Embassy with:

  1. The confirmation email message;
  2. A valid ID;
  3. The form “Atto di assenso” duly filled in but not signed (it will be signed at the Embassy);
  4. The cash for the applicable consular fee (Art. 19, of the Tabella dei diritti consolari).

 

VALIDITY OF THE CONSENT

The day of the appointment you will receive the original copy of the document while a scanned copy will be sent via email to the Office where the travel document/ID will be issued. Please note that the consent is valid for 6 months from the date it was signed.
